I think a child should be seen alone in the room without their parents starting at 12 or 13 years of age. So usually at the 11 or 12-year old visit, I'll say to the child with the parent in the room, "You know, next year I'm going to examine you without your parent in the room and just wanted to give you a head's up." And the following year when they come in for their checkup, I talk to the parent and the child together and then at some point I say to the parent, "I'm going to ask you to leave the room for a few minutes while I do this examination." And usually that's fine with the child. I would never ask the child, "Is it okay if your parent leaves the room?" because I wouldn't want to put the child on the spot. And if they said no, or if they said yes, I sort of make that executive decision.
