The mode of delivery when delivering twins, begins with a detailed conversation between the parents and the obstetrician. Factors to be considered are the presentation of the twins, preference of the parents, and the experience of the obstetrician. 60 percent of twins are born via cesarean. However, a vaginal birth can occur if the head presentation is either vertex/vertex, or head down/head down, or vertex/non-vertex, which would be head down/breech, or transverse lie. A trial of a vaginal delivery can happen without any complications, as long as the first twin is delivered and the second twin is continuously monitored. The second twin can be delivered anywhere from minutes to hours following the birth of the twin. As long as they are safe, there is no time limit in delivery between the first and the second twin.
