I think it's important to both have a midwife and a doula at home birth because there's a lot going on. The midwife primarily takes care of the birth, of course she helps you with the contractions and the laboring; but the doula, really does support you as a mother. She can help comfort you and get you through a lot of the labor and contractions. During the actual birth, the midwife is handling most of the birthing, but she needs an assistant to help her. At the same time, birth can be a long process, and they do need to support each other and take breaks.
